<u><em><strong>Purpose</strong></em></u><br>
The Clinical Case Educator is an important role in our team. The nurse is passionate and is committed to provide a high level of patient care and counseling patients enrolled in the Medication Therapy Program. The ideal candidate has great communication skills, problem solving skills, and must be self-motivated. This person will be driven to help team members to achieve the mission of providing high level of care to patients.<br>

<u><em><strong>Essential Duties and Responsibilities</strong></em></u><br>
<br>
&bull; Shift Details: Monday -Friday 9:00 am - 6:00 pm (occasional Saturdays)<br>
&bull; Counsel and educate new/existing patients on specialty drug therapies<br>
&bull; Adherence to contracted and accrediting regulations as indicated by drug specification<br>
&bull; Complete, thorough and accurate documentation utilizing good documentation practice and compliance to industry standards and regulations<br>
&bull; Discuss potential/current side effects with patients to educate about treatment alternatives and facilitate ongoing therapy adherence<br>
&bull; Identify and report adverse events (AEs) and product complaints (PCs) to the manufacturer, FDA, and/or other governing authorities<br>
&bull; Specialty drug verification and education. Provider/patient conduit to support therapeutic longevity.<br>
&bull; Adherence to patient management programs through call cadence schedules, medication management programing, drug counseling and provider collaboration.<br>
&bull; Maintenance of patient demographic records, medical history and records in accordance to federal HIPAA standards<br>
&bull; Collaboration with Pharmacy Support teams to ensure timely and accurate delivery of specialty drugs<br>
<br>
<u><em><strong>Qualifications/Requirements</strong></em></u><br>
<br>
&bull; Registered Nurse with current unrestricted compact state license or primary NC licensure<br>
&bull; Associates, Bachelor or higher degree in Nursing or accredited nursing diploma<br>
&bull; Ability to obtain licensing in all states and United States territories serviced within 3 months of employment through Biologics supported programming (minimal application guidelines apply)<br>
&bull; 4+ years of nursing experience with at least 1 year of acute, inpatient care/oncology preferred<br>
&bull; Strong Computer skills, including e-mail, word processing, spreadsheet and web-based programs<br>
&bull; Effective orally and in writing communication, experience with motivational interviewing preferably<br>
&bull; Ability to read, analyze and interpret practice standards, professional journals, technical procedures and government standards and regulations<br>

Ensuring that progress reports are made to the physician when the clients condition changes. Transcribing or ensuring transcription of physician phone orders and other medically related reports. Complete daily, monthly, quarterly and annual nursing documents as/ if required. Ensuring the routine documentation of individuals health status in nurses notes. Ensuring that furnishings, equipment, and supplies are maintained within guidelines established to prevent the spread of disease. Directing and supervising the activities of the licensed practical / vocational nurse as needed. Ensure that appointments for health-related issues are scheduled, completed and that appropriate response to findings are implemented in a timely manner. Maintain communication with inpatient nursing and discharge coordinators when individuals served are hospitalized. Provide new employee training and annual recertification training for health and safety related topics. Ensure unlicensed staff are trained on person-specific issues as they relate to each individuals health maintenance, safety, and disease prevention needs.<br>

Key Responsibilities

<ul>
&#9;<li>Maintain inventory of surgical instrumentation and supplies to support efficient operations
&#9;<li>Proficiency with Decontamination,Sterilization,&nbsp;Endoscopy scopes and&nbsp;Assembly and wrapping
&#9;<li>Manage and maintain both purchased and loaner implant sets
&#9;<li>Collaborate effectively with surgical staff, physicians, and internal departments
&#9;<li>Sterilize instruments, equipment, and supplies using autoclaves and other sterilization systems
&#9;<li>Load and operate sterilization equipment according to manufacturer guidelines and regulatory standards
&#9;<li>Accurately document sterilization cycles and verify parameters
&#9;<li>Assemble instrument trays, sets, and procedure kits according to standardized count sheets
&#9;<li>Inspect, clean, disinfect, and reassemble surgical instruments and equipment
&#9;<li>Ensure all instruments are functioning properly and report deficiencies
&#9;<li>Pull and prepare case carts and surgical supplies for scheduled procedures
&#9;<li>Maintain clean, organized, and compliant work areas
&#9;<li>Monitor and restock inventory as needed

Qualifications

This onsite Clinical Pharmacist role focuses on delivering high-touch pharmacotherapy counseling and coordination for oncology patients. By integrating clinical expertise with patient education and regulatory compliance, the pharmacist plays a critical role in driving treatment success, ensuring continuity of care, and supporting clinical operations and intake. The ideal candidate excels in patient communication, multitasking, and collaborative clinical coordination.<br>
<strong>KEY RESPONSIBILITIES</strong><br>

            <description><![CDATA[<br>CornerStone Medical is in search of&nbsp;Registered Nurses to provide&nbsp; effective&nbsp;therapy to Chronic Hemodialysis&nbsp;patients&nbsp;in the&nbsp;clinic &&nbsp;hospital settings&nbsp;in Grapevine&nbsp;TX.&nbsp;<br>
&nbsp;<br>
*The hemodialysis nurse position will consist of&nbsp;seeing 2 patients at a time and 7 to 8 patients a day.<br>
<br>
<strong>Shifts:</strong>&nbsp;&nbsp;3-shifts (Daily). starting at 6am or 7am&nbsp;<br>
<strong>Pay:</strong>&nbsp;$45&nbsp;- 48/hr. DOE&nbsp;<br>
<br>
<strong><u>PRINCIPAL DUTIES AND RESPONSIBILITIES:</u></strong><br> 

<ul>
&#9;<li>All duties and responsibilities are expected to be performed in accordance with company standards of nursing practice, state and federal regulations.
&#9;<li>Performs all essential functions under the direction of their direct supervisor and with guidance from their preceptor or their Acute Care Coordinator (ACC) for the first 6 months of employment.&nbsp;
&#9;<li>Performs ongoing, systematic collection and analysis of patient data pre â during â post treatment for assigned patients and documents in the patient medical record, adjusts or modifies the treatment plan as indicated and notifies the Nephrologist, patientâs primary nurse and others as may be indicated.&nbsp;
&#9;<li>Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input including but not limited to the Nephrologist and patientâs primary nurse.
&#9;<li>Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed.
&#9;<li>Directs and provides safe effective patient care for assigned patients as prescribed for all modality specific treatment procedures.&nbsp;

<br><strong><u>EDUCATION:</u></strong><br>
&nbsp;<br> 

<ul>
&#9;<li>Graduate of an accredited School of Nursing
&#9;<li>Current appropriate state licensure in good standing
&#9;<li>Must meet the practice requirements in all states in which he or she is employed
</ul>

<strong><u>EXPERIENCE AND REQUIRED SKILLS:</u></strong><br>
&nbsp;
<ul>
&#9;<li>Minimum of six (6) months hemodialysis experience&nbsp;
&#9;<li>Minimum of one (1) year medical/surgical or ICU nursing experience preferred
&#9;<li>BLS certification from American Heart Association
&#9;<li>ACLS certification from American Heart Association
&#9;<li>OSHA, infection control and continuing education requirement.
&#9;<li>Successful completion of on-going training and education as assigned or required
&#9;<li>Must provide&nbsp;immunization records
&#9;<li>Must pass background check and drug test&nbsp;

-When performing clinical duties, this role functions under the clinical supervision of the provider in physician practice or clinic setting, assisting in the preparation and delivery of ambulatory patient care as directed by the provider(s) and other licensed clinical personnel.<br />

-Clinical duties are skillfully and consistently performed per established clinical policy and procedure.<br />
<br />
- Clerical duties are performed under the direct supervision of the practice manager or supervisor, including assisting with the overall management of daily patient flow: greeting patients, patient check-in procedures, scheduling appointments and referrals, promptly managing phone calls, and patient check-out procedures.<br />
<br />
-Data entry skills are required of all aspects of this role, as each patient encounter is entered into an electronic medical record (EMR).<br />
