<u><em><strong>Purpose</strong></em></u><br>
The Clinical Case Risk Evaluation and Mitigation Strategy (REMS) Clinical Nurse Educator is an important role in on our team. The nurse is passionate and is committed to provide a high level of patient care and counseling patients enrolled in the Medication Therapy Program. The ideal candidate has great communication skills, attention to detail, problem solving skills, and must be self-motivated. This person will be driven to help team members to achieve the mission of providing high level of care to patients.<br>
<br>
<u><em><strong>Essential Duties and Responsibilities</strong></em></u><br>
-Counsel and educate new/existing patients on specialty drug therapies existing patients on REMS FDA-mandated medications. For some of these medications, as a part of the REMS program, the nurse will read a list of statements to the patient verbatim.<br>
-Help ensure patients and their caregivers are educated on their current therapy, will help ensure medication adherence, will schedule medication refill deliveries and collect copays, if applicable<br>
-Adherence to contracted and accrediting regulations as indicated by drug specification.<br>
-Complete, thorough and accurate documentation utilizing good documentation practice and compliance to industry standards and regulations<br>
-Discuss potential/current side effects with patients to educate about treatment alternatives and facilitate ongoing therapy adherence<br>
-Identify and report adverse events (AEs) and product complaints (PCs) to the manufacturer, FDA, and/or other governing authorities<br>
-Specialty drug verification and education. Provider/patient conduit to support therapeutic longevity.<br>
-Adherence to patient management programs through call cadence schedules, medication management programing, drug counseling and provider collaboration.<br>
-Maintenance of patient demographic records, medical history and records in accordance to federal HIPAA standards<br>
-Collaboration with Pharmacy Support teams to ensure timely and accurate delivery of specialty drugs<br>
<br>
<u><em><strong>Qualifications/Requirements:</strong></em></u><br>
<br>
-Registered Nurse with current unrestricted compact state license or primary NC or TX licensure<br>
-Associates, Bachelor or higher degree in Nursing or accredited nursing diploma<br>
-Ability to obtain licensing in all states and United States territories serviced within 3 months of employment through Biologics supported programming (minimal application guidelines apply)<br>
-5 or more years of nursing experience with at least 1 year of acute, inpatient care or outpatient care preferred<br>
-Computer skills, including e-mail, word processing, spreadsheet and web-based programs<br>
-Effective orally and in writing communication, experience with motivational interviewing preferably<br>
-Ability to read, analyze and interpret practice standards, professional journals, technical procedures and government standards and regulations<br>

&bull; Utilized pharmacy systems technology and automation to optimize workflow and streamline<br />
medication dispensing process.<br />
&bull; Maintained inventory levels, conducted regular audits, and liaised with suppliers to ensure<br />
availability of essential medications, resulting in reduced delays in patient care.<br />
&bull; Collaborated with pharmacists to ensure accurate and timely preparation and distribution of<br />

<strong>Job Summary</strong>:<br />
The ideal candidate will be able to accurately and efficiently fill prescription requests in an automated refill setting. Under the supervision of a Pharmacist, the Pharmacy Technician prepares, packages and distributes medications in a fast-paced production environment. Maintains continuous, consistent workflow in order to meet daily production goals; and has the ability to work productively either independently or as a team.<br>

<strong>Key Responsibilities/Essential Functions:</strong><br />

1. Medication Dispensing:<br />
&bull; Prepare and label prescription medications for dispensing.<br />
&bull; Measure, count, and package medications according to prescription orders.<br />
&bull; Replenish medication in automated cells to full capacity; opening large quantities of bottles and<br />
cases of product.<br />
&bull; Ensure the proper labeling and instructions for patient use on medication containers.<br />
&bull; Maintain an organized and clean dispensing area.<br />

2. Inventory Management:<br />
&bull; Conduct regular stock rotation to prevent expiration of medications.<br />
<br />
3. Compliance and Regulations:<br />
&bull; Adhere to all federal, state, and local pharmacy laws and regulations.<br />
&bull; Maintain patient confidentiality and HIPAA compliance at all times.<br />
&bull; Assist in record-keeping and documentation of prescription and medication-related information.<br />

<strong>Minimum Qualifications</strong><br>

<br>-Serves as a Liaison between patients, caregivers, home health agency nurses, hospice nurses, and the prescribing physician/clinician for orders related to the services provided by Biologics, Inc. to ensure all stakeholders receive outrageous customer service<br>

Assists the Pharmacist in final checking of filled prescriptions, which may include stating the medication for RPh final review and/or packaging of medication post RPh final review and coordinating accurate and timely delivery to the customer. Assists Pharmacy Management in maintaining good inventory practices which may include receiving and rotating incoming medications and supplies, posting lot numbers, expiration dates and assisting in physical inventory. Works with Technician Supervisor in maintaining a clean/safe work environment, checking equipment for calibration, safety, and cleanliness<br>

Duties of this healthcare role will include, but are not limited to: Observing, assessing, planning, implementing and evaluating nursing support for well, chronic or acutely ill individuals in a community or small facility setting. Working closely with other health professionals/providers in planning, implementing, and evaluating programs and services to address the health needs of persons served. Coordinate communications and actions of various disciplines as it relates to health and safety supports. Ensuring the ordering, reviewing, monitoring, discontinuation and, if necessary, the disposal of medications as prescribed by the physician. Ensure that there are policies in place that address medication administration, medication errors, medication storage / access and medication destruction, and that these policies are reviewed at least annually and revised as necessary. Ensuring that progress reports are made to the physician when the clients condition changes. Transcribing or ensuring transcription of physician phone orders and other medically related reports. Complete daily, monthly, quarterly and annual nursing documents as/ if required. Ensuring the routine documentation of individuals health status in nurses notes. Ensuring that furnishings, equipment, and supplies are maintained within guidelines established to prevent the spread of disease. Directing and supervising the activities of the licensed practical / vocational nurse as needed. Ensure that appointments for health-related issues are scheduled, completed and that appropriate response to findings are implemented in a timely manner. Maintain communication with inpatient nursing and discharge coordinators when individuals served are hospitalized. This role includes reviewing referrals for appropriateness (drug, dose, route, labs), completing new patient starts and refills, performing medication reconciliation and patient counseling, monitoring lab-dependent therapies, and coordinating care with providers and nursing. The pharmacist is also responsible for accurate prescription entry, documentation, and claim adjudication, as well as preparing and verifying sterile compounded medications, ensuring proper labeling, lot tracking, and compliance with USP standards. Additional responsibilities include supervising pharmacy technicians, supporting production and delivery workflows, maintaining inventory, and ensuring timely, safe medication dispensing and delivery in a fast-paced home infusion environment.<br>

Coder will work collaboratively with various Health departments, to ensure accurate and complete physician documentation to support accurate billing and reduce denials. Coder will also assist in other areas of the department, as requested by leadership. <strong><u>PRINCIPAL DUTIES AND RESPONSIBILITIES:</u></strong><br> 

<ul>
&#9;<li>All duties and responsibilities are expected to be performed in accordance with company standards of nursing practice, state and federal regulations.
&#9;<li>Performs all essential functions under the direction of their direct supervisor and with guidance from their preceptor or their Acute Care Coordinator (ACC) for the first 6 months of employment.&nbsp;
&#9;<li>Performs ongoing, systematic collection and analysis of patient data pre â during â post treatment for assigned patients and documents in the patient medical record, adjusts or modifies the treatment plan as indicated and notifies the Nephrologist, patientâs primary nurse and others as may be indicated.&nbsp;

<strong>ROLE IMPACT</strong><br>
The primary responsibility of the Clinical Pharmacist is to provide oversight<br>
and safety for all daily operations of the assigned patient care team, including management of assigned pharmacy staff, clinical support to patients, and ensuring that all prescriptions are dispensed in accordance with state, federal and board of pharmacy laws.<br>

<strong>KEY RESPONSIBILITIES</strong><br>
-Conducts a comprehensive clinical review of pharmacotherapy for each patient including drug utilization, counseling, compliance, drug interaction screening, side effect profiling and drug administration support to ensure the patientâs treatment plan is in line with evidence-based recommendations<br>

-Documents results of pharmacotherapy review in patient chart<br>
so that the nursing team performing follow-up assessments are aware and educated<br>

-Counsels patients on new drug therapy in accordance with clinical management models so that patients are educated on the specifics of their planned treatment<br>

-Completes REMS training and follows program SOPs accordingly<br>

-Coordinates patient care with the referring clinician, including but not limited to, social, psychological and physical needs that warrant customized services by Biologics, Inc. (i.e. pre-filled syringes, color coded labeling for medication identification, etc.) and documenting these services in their progress note<br>
so that the fulfillment and distribution teams are aware of additional materials to add<br>
<strong>MINIMUM QUALIFICATIONS</strong><br>
