Job Description

We are hiring an RN Case Manager for a state-of-the-art rehabilitation facility in Arlington TX. We are currently seeking a compassionate and experienced Registered Nurse Case Manager to coordinate and oversee the care of our patients throughout their rehabilitation journey. 

Hours: Mon - Fri  8:00a.m - 5:00p.m. 

Responsibilities:
  1. Meets with patient upon admission to discuss rehabilitation process and introduces role of case coordinator. Conducts and documents initial patient assessment within one working day
  2. In collaboration with the health care team, assesses patient population for appropriate level of care across the care continuum; identifies risk factors such as clinical, psychosocial, and/or financial indicators; facilitates the involvement of support services to promote optimal clinical and financial outcomes.
  3. Reviews goals with patient’s family (with patient’s permission) and third party payer. Maintains accurate up-to-date documentation in the medical records of patient and family contacts, communications with payers and other services performed on behalf of patient and family.
  4. Coordinates, evaluates, documents and reviews interdisciplinary team goals as they pertain to the plan of care and transition plan.
  5. Coordinates team conferences, and documents in the medical record a summary of the team conference. Serves as the primary family contact (with patient permission) for the treatment team, communicating within 2 business days of the team conference. Also, as appropriate, updates the external case manager and/or payor source regarding progress towards goals, plan and patient’s post-acute care needs (as requested by payer source).
  6. Calls patient or family as needed post-transition to ensure planned recommendations are occurring and to assess if additional services are needed.
  7. In collaboration with the Utilization Review Team, monitors and ensures needs of per diem negotiations, recertification, concurrent reviews and timely reports.
  8. Models quality patient/customer service by: demonstrating a willingness to serve all customers at all times, maintaining an awareness of patient/customer needs and responding to those needs, maintaining confidentiality of all patient information, and functioning in a collaborative manner with other care-givers to ensure quality patient care.
  9. Performs other duties as assigned
Qualifications:
  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing or related field preferred.
  • Current unrestricted Registered Nurse (RN) license in Texas
  • Certification in Case Management (e.g., CCM) preferred.
  • Minimum of 1 year of clinical experience in rehabilitation desired -3 years experience in a hospital setting 
  • Proficiency in electronic health records (EHR) and Microsoft Office applications.
