$45Hr -$48hr Job Summary:
A Rehabilitation Registered Nurse (Rehab RN) provides care to patients recovering from illness, injury, or surgery, helping them regain independence and improve their quality of life. The Rehab RN works as part of an interdisciplinary team to create and implement individualized care plans tailored to patients’ physical, emotional, and medical needs.
Key Responsibilities:

Patient Care & Assessment:
- Assess and monitor patients’ physical and emotional well-being.
- Develop and implement individualized care plans.
- Provide wound care, pain management, and medication administration.
- Assist with mobility training, transfers, and adaptive techniques.

Rehabilitation & Therapy Support:
- Collaborate with physical, occupational, and speech therapists to support patient progress.
- Educate patients and families on adaptive techniques and home care strategies.
- Encourage patient participation in therapy exercises and activities.

Collaboration & Communication:
- Work closely with physicians, therapists, and case managers to ensure continuity of care.
- Document patient progress, treatments, and changes in condition accurately.
- Provide emotional support and guidance to patients and their families.

Safety & Compliance:
- Follow infection control, patient safety, and facility protocols.
- Maintain a safe environment and prevent falls and complications.
- Stay updated on best practices in rehabilitation nursing.
- Education: Associate’s or Bachelor’s Degree in Nursing (ADN or BSN).
- License: Active Registered Nurse (RN) license in the state of practice.
- Certifications: Basic Life Support (BLS);
- Immunizations as required by facility
- Experience: Prior experience in rehab, med-surg, or long-term care is a plus.
- Strong assessment and critical-thinking skills.
- Excellent communication and teamwork abilities.
- Compassionate and patient-centered approach.
- Ability to support patients in regaining independence.
- Inpatient rehab facilities, acute care hospitals, skilled nursing facilities, or outpatient rehab centers.
- May require lifting, standing, and assisting patients with mobility.
