M–F 8:00am–4:30pm CST; on-call weekends & after hours
$55/hr
ROLE IMPACT
This Registered Nurse position supports home health clients in the Shelby, TN area by delivering skilled nursing care, assessing individual health needs, and ensuring effective case management. Success in this role means delivering high-quality, person-centered care, supporting preventive health measures, and collaborating across disciplines to ensure comprehensive care planning and delivery.
KEY RESPONSIBILITIES
• Conduct assessments and implement care plans for clients in home health settings
• Monitor, evaluate, and document health status, medication management, and physician communications
• Coordinate interdisciplinary communications and follow up on health-related appointments and outcomes
• Provide supervision to LPNs and unlicensed staff, ensuring adherence to clinical protocols and health/safety policies
• Deliver training to staff on individual-specific health maintenance, safety, and disease prevention
LICENSES & CERTIFICATIONS
• Registered Nurse (RN) – State of Tennessee, active and in good standing
• BLS certification (Basic Life Support)
CLINICAL SKILL SET
• Wound care management
• OASIS assessment and documentation
• Home health care planning and delivery
• Medication administration and monitoring
• Disease prevention and health education
• Documentation using electronic medical records
• Staff training on individual health protocols
• Care coordination with physicians and hospitals
• Infection control and medical equipment oversight
• Health status monitoring and progress reporting
PREFERRED SKILLS
• 1+ year experience in home health or community-based nursing
• Demonstrated competency in wound care and OASIS (testing on Day 1)
• Strong organizational and interdisciplinary communication skills
