Job Description
Brenham, TX | 13-Week Contract | $65/Hour
CornerStone Medical is actively seeking a dedicated Physical Therapist (PT) for a 13-week travel assignment with a respected healthcare facility in Brenham, Texas. This is an excellent opportunity to work with a collaborative rehabilitation team while making a meaningful impact on patient recovery and quality of life.
Pay Rate: $65.00 per hour
Schedule
Monday – Thursday: 8:00 AM – 5:00 PM
Friday: 8:00 AM – 4:00 PM
Position Summary
The Physical Therapist provides comprehensive patient care through evaluation, treatment planning, implementation of therapeutic interventions, patient and family education, and discharge planning. The goal is to maximize patient function, independence, and overall quality of life while delivering evidence-based care.
Key Responsibilities
Patient Care
- Perform comprehensive patient evaluations and assessments
- Develop individualized treatment plans based on patient needs and goals
- Implement and modify therapy programs to optimize outcomes
- Monitor patient progress and adjust treatment strategies as needed
- Facilitate safe and effective discharge planning
- Complete timely, accurate, and thorough clinical documentation
- Enter charges accurately and understand reimbursement requirements
- Maintain compliance with facility and regulatory standards
- Effectively communicate with patients, families, physicians, and interdisciplinary team members
- Educate patients and caregivers on treatment plans, exercises, and recovery goals
- Foster positive relationships with patients and healthcare team members
- Supervise support staff, students, and volunteers as assigned
- Participate in continuing education and evidence-based practice initiatives
- Share knowledge and mentor colleagues to support clinical excellence
- Adhere to all state, national, and organizational regulatory guidelines
- Minimum of 1 year of Physical Therapy experience required
- Strong clinical reasoning and problem-solving abilities
- Excellent interpersonal, communication, and teamwork skills
- Commitment to delivering exceptional patient-centered care
- Current Physical Therapist License (Texas or Compact State License)
- BLS Certification (AHA Only)
